Causes of a tooth gap: How does a diastema actually develop?

Tooth gaps, especially in the anterior region, are often caused by

  • Inherited differences in tooth and jaw size
  • A lip frenulum that is too large (frenulum labii)
  • Tooth loss or failure of individual teeth to grow
  • Tooth migration due to periodontitis
  • Differences in tooth size - also known as Bolton discrepancy (more information on this further down in the article)

Closing a tooth gap: What treatment options are there?

1. orthodontic treatment: closing a tooth gap with braces

With modern orthodontic appliances such as fixed braces or invisible aligners, a gap between teeth can be corrected gently and over the long term. The teeth are slowly moved into the ideal position, which closes the gap.

Advantages:

  • Preservation of the natural tooth structure
  • Permanent and stable solution
  • Also suitable for complex malocclusions

Special feature: Tooth gap due to Bolton discrepancy

An often overlooked reason for gaps in the front teeth is the so-called Bolton discrepancy - a size mismatch between the upper and lower teeth, named after the American orthodontist Dr. Wayne A. Bolton.

What does this mean?
If, for example, the lower front teeth are proportionally too large or the upper front teeth are too narrow, a gap is created despite correctly positioned teeth, usually between the upper central incisors.

Treatment for Bolton discrepancy:

  • Orthodontic: space distribution and, if necessary, interproximal enamel reduction (stripping)
  • Restorative: build-up of teeth that are too small with composite or veneers
